Centre for crisis management, diagnostic reassessment and detoxification «VILLA NINA»

INTRODUCTION

Villa Nina was set up as specialist unit in response to the complexity of the conditions of multi-drug abuse. The facility is located in Longana, a rural area near Ravenna. The programme is designed for 20 residents, both male and female; 15 places are dedicated to the Intensive Module and the remaining 5 to the Zip Module.

Young multi-drug abusers of both genders, aged between 18 and 30 years. Adults (up to 40 years) with personality characteristics and/or methods of substance intake for which it is possible to define treatment goals consistent with the therapies the facility provides. Requests to get into the Villa Nina programme are based on the bio-psychosocial factors of the patient that make outpatient treatment impossible. The benefits of the facility's therapies are specifically aimed at people with personality characteristics and/or methods of substance intake and deal with:

  • assessment of sobriety or drug free conditions;
  • need to learn and build up effective strategies to avoid substances and to control risk situations. Consistent with the specific characteristics and objectives of the facility and the short duration of the courses, the health service which has sent the patient to the centre must be capable of providing follow up treatment after the assessment stage.

The residential programme has two modules - Zip and Intensive - accommodating a total of 20 residents.

ZIP Module

(STAY - Maximum 30 days)

The Zip Module is designed to approach and treat conditions in a 30 day period, which will then be followed up with alternative treatment.

  • Scaling down with substitute drugs and psychotropic medication
  • Induction and stabilisation of drug therapy

Psychic and social area

  • Diagnostic assessment of resident's psychosocial functioning
  • Evaluation of the family context
  • Crisis management (aggravation of family conflict, risk of job loss, relapse, difficulties encountered in other facilities, etc.)

The Zip Module can be a prerequisite to the Intensive Module.

Intensive Module

(STAY - Maximum 3 months)

The Intensive Module is a specialised therapy in response to the complexity of the conditions of multi-drug abuse. Residents are multi-drug abusers between the ages of 18 and 30 years and adults up to 40 years with a history of alternate use of different substances or multi-drug abuse, with personality traits related to sensation seeking traits.

OBJECTIVES

Health area

  • Detoxification and withdrawal from alcohol, heroin and benzodiazepines
  • Scaling down with substitute drugs and psychotropic medication
  • Induction and stabilisation of drug therapy
  • Health improvement

Psychic and social area

  • Diagnostic assessment of resident's psychosocial functioning
  • Evaluation of the family context
  • Crisis management (aggravation of family conflict, risk of job loss, relapse, difficulties encountered in other facilities, etc.)
  • Increased motivation to change
  • Relapse prevention
  • Re-evaluation of the treatment plan in progress
  • Increase/restore capability in self-care and personal relationships

TOOLS FOR BOTH MODULES

Clinical area: Individual and family clinical interview, psycho-educational meetings/groups, Tests (EuropASI, SCID, MMPI), DBT Groups (only for the Intensive Module).
Health area: medical consultation, treatment administration, health education.

Methodology

The key elements of the therapeutic intervention are psychotherapy according to dialectical-behavioural and systemic-relational approaches, psycho-education and musical and interdisciplinary laboratories.

The dialectical-behavioural intervention is aimed at the acquisition of effective strategies in reducing behaviour that stimulates the conduct of abuse, craving reduction, avoidance of stimuli associated with the substance, increased ability to manage emotions, reduced impulsivity and enhancement of coping strategies.

The systemic-relational intervention aims at improving communication between family members and bringing out the resources of the system so that it can act as a container and support the individual.

The psycho-educational intervention aims at increasing the level of information and awareness of the physical and mental condition of the person and the possible progress; increasing the level of information on drug therapy implemented and its effectiveness, on any side effects and consequences of non-adherence to the same.

The Tititom method (protected by international patent) aims at the development of personal creativity and the individual's planning and construction skills by means of complex musical practices.

Exclusion Criteria

Exclusion Criteria

  • Strongly affected health conditions: - diseases that require continuous monitoring of the health status and/or intercurrent drug therapy (e.g. decompensated liver cirrhosis, decompensated diabetes mellitus, disabling cardiac and/or respiratory problems, etc.) - Diseases that interfere with the ability to reach objectives and carry out daily activities (e.g. hepatic encephalopathy, severe cognitive impairment, outcome of cerebral vascular diseases etc.), motor disabilities.
  • History of delirium tremens and/or seizures in abstinence of alcohol. 
  • Presence of psychiatric disorders that interfere with the ability to reach objectives and daily activities such as persistence of psychotic traits and/or grossly dysfunctional behaviour. 
  • Incompatibility with other residents.

"Special" cases

In cases of minors (minimum 17 years) and the presence of restrictive legal measures, the Villa Nina treatment team shall evaluate the inclusion on the basis of compatibility with other residents, and the possible interference with the achievement of the goals of the facility's treatment plan.

Service operators, may however, submit a request for inclusion in situations they deem appropriate, and each request will always be evaluated by the Villa Nina treatment team.

At the end of the course, the Villa Nina treatment team will produce a report based on the results achieved in the facility (the results of the diagnostic evaluation if required, progress of the project within the facility, achievement of objectives and possible problems, achievements in the musical workshop) and instructions on the continuation of the treatment plan outside of the facility.

From 2001 to 2008 Dr. Carrozzo carried out clinical work with individuals and families presenting problems related to the use of alcohol, eating disorders, pathological gambling and dual diagnosis patients at Ser.T. Cesena. Since 2002 she has been holding training courses for health workers.

The nursing service is guaranteed from 7.30 to 22.30 with the presence of 3 nurses and a medical toxicologist.

Since 2010 Dr. Guerzoni has been performing clinical and teaching activities as well as research at the Medical Toxicology Unit of the University Hospital Polyclinic of Modena.
